Instructions for Large 10-Batch Investigation Form

Purpose:

This form allows you to submit up to 10 asset search investigations simultaneously. The form is specifically designed to accommodate searches for individual debtors and their corresponding businesses.

Structure:

Each section contains TWO separate search forms:

• Individual Debtor Asset Search Form
• Corresponding Business Asset Search Form

How to Complete the Form For Each Debtor:

1. Complete the Individual Debtor Asset Search Form with all available information about the person.
2. If the individual owns or is associated with a business that also requires investigation, complete the corresponding Business Asset Search Form immediately below it.
3. If no business search is needed, simply leave the Business Asset Search Form blank and proceed to the next Individual Debtor section.

Example Scenarios:

• Scenario 1: If you need to search assets for both John Smith and his business “Smith Enterprises,” complete both forms in that section.
• Scenario 2: If you only need to search assets for Jane Doe with no associated business, complete only her Individual Debtor form and leave the Business form blank.

Important Notes:

• You only need to fill out the forms relevant to your investigation needs
• Each Individual/Business pair counts as one of your 10 available searches
• You may submit fewer than 10 investigations if needed
